Back pain is the leading cause of disability worldwide, yet most episodes resolve without any imaging or specialist intervention. What I look for when assessing back pain remotely is whether there are 'red flags' - signs that suggest something more serious than a simple muscle or ligament strain. These include loss of bladder or bowel control, numbness in the groin area (cauda equina syndrome, a surgical emergency), progressive leg weakness, or back pain with fever. In the absence of these red flags, the evidence is clear: staying gently active recovers faster than bed rest. A medical certificate allowing modified duties or a few days' rest is often the most practical step, especially for physically demanding work. Most back pain peaks in the first 48-72 hours and improves steadily over 2-4 weeks.
Most acute back pain is managed with simple analgesics, brief relative rest, and early return to activity. The eTG recommends paracetamol as first-line; NSAIDs are added if paracetamol alone is insufficient. Muscle relaxants are not recommended routinely due to sedation risk and limited evidence. For chronic back pain (greater than 3 months), a multimodal approach including physiotherapy, exercise, and psychological support has the strongest evidence base.

Acute back pain: most improvement within 2-4 weeks. 90% of cases resolve within 6 weeks. Some residual stiffness or mild discomfort may persist for several weeks beyond that.
Return to work
For desk-based work, most people can return within a few days with ergonomic adjustments. For physical or manual labour, a graded return over 1-2 weeks is typical. Staying off work completely for more than a few days usually slows recovery rather than helping it.
When to reassess
See a doctor urgently if you develop numbness in the groin, loss of bladder or bowel control, or progressive weakness in the legs. Otherwise, reassess if pain hasn't improved after 4-6 weeks, or if it's worsening despite self-care.
